These are the moments when businesses without in-house safety expertise need clear, experienced help quickly, before decisions made in the first few days make matters worse.

Accident and near-miss investigation

When something goes wrong, you need to understand what happened and why. Our consultants investigate accidents and near misses methodically: establishing the facts, speaking to the people involved, reviewing the risk assessments and procedures that applied, and identifying the underlying causes rather than stopping at the obvious ones.

Findings are documented properly in a structured investigation report, the kind insurers, clients and enforcing authorities expect to see. From there we advise on corrective actions that prevent recurrence: practical improvements to how the work is planned and done, not blame exercises.

RIDDOR reporting

RIDDOR is the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 2013. It requires employers, the self-employed and those in control of work premises to report certain work-related incidents to the HSE and to keep records of them.

Reportable incidents include work-related deaths, specified injuries such as fractures (other than to fingers, thumbs and toes), amputations and crush injuries, injuries that leave a worker unable to do their normal duties for more than seven consecutive days, injuries to members of the public taken directly to hospital, certain occupational diseases, and dangerous occurrences: specified near misses such as scaffold collapses or failures of lifting equipment, even where no one was hurt.

The deadlines are tight. Deaths and specified injuries must be reported without delay, and over-seven-day injuries within 15 days of the accident. Judging what is and is not reportable is not always straightforward, so we handle it for you: assessing whether an incident meets the reporting threshold, submitting the report to the HSE, and making sure your records are kept as the Regulations require.

Support during HSE inspections

We support you during HSE or local authority inspections, whether the visit is planned or an inspector has arrived unannounced. That means:

  • Preparing your site, documents and people ahead of a planned visit
  • Being present on the day where needed
  • Responding to inspector findings, notifications and enforcement notices
  • Advising on fee-for-intervention exposure and how to limit it
  • Planning and evidencing the corrective actions inspectors expect to see

Where an inspector identifies a material breach, the HSE recovers the cost of its time under its fee-for-intervention scheme, so resolving issues promptly and demonstrably is not just good practice, it directly limits what an inspection costs you.

After the event: putting it right

An investigation or inspection is only worth what changes afterwards. We help you close out corrective actions properly: updating your risk assessments, method statements and policies so they reflect what was learned, arranging refresher training where a gap contributed to the incident, and following up with audits that check the improvements have stuck.

Where it is useful, we fold the lessons into ongoing retained support through our consultancy packages, so the same issue does not come back.

What is RIDDOR and which incidents must be reported?

What is HSE fee for intervention?

Fee for intervention (FFI) is the HSE’s cost-recovery scheme. When an inspector finds a material breach of health and safety law, the HSE charges for the time it spends identifying the breach and helping to put it right, at an hourly rate. Dealing with issues quickly, and being able to show you have, is the most effective way to limit that exposure.

Will an investigation blame our staff?

No. The purpose of an investigation is to understand what happened and why, so it does not happen again. That usually means looking at how the work was planned, equipped and supervised rather than singling out individuals. Findings are documented factually, and the focus is always on practical corrective actions.
